Open rhinoplasty is a way of carrying out nose reshaping surgery in which the surgeon makes a small incision across the columella, the strip of skin that separates the nostrils, in addition to incisions hidden inside the nose. This lets the skin of the nose be lifted so the underlying bone and cartilage can be seen and worked on directly. The alternative, closed rhinoplasty, is performed entirely through incisions inside the nostrils, with no external cut and no visible scar.
The main advantage of the open approach is direct visibility. By exposing the structures fully, the surgeon can see and adjust them precisely, which is why it is often chosen for complex reshaping, for revision surgery where the anatomy has already been altered, or where detailed grafting is needed. The trade off is a small scar across the columella, which usually fades to become hard to see, and often a longer period of swelling at the tip of the nose compared with the closed approach.
Neither approach is simply better. The right choice depends on what your nose needs and on the surgeon's training and preference. Both are real surgery on a structure that affects breathing as well as appearance, both carry genuine risks, and the result that lasts is shaped far more by the surgeon's skill and judgement than by the label open or closed. Rhinoplasty is also widely regarded as one of the harder procedures to get right, and revisions are not uncommon.
Weigh the surgeon, not just the method.
If a clinic abroad proposes open rhinoplasty, ask why that approach suits your nose, what is planned, and how the surgeon expects the result and the recovery to look. Because rhinoplasty is technically demanding and the swelling can take many months to fully settle, ask how revisions are handled if you are unhappy, who pays, and how that would work once you are back home and far from the surgeon.
The choice between open and closed is a detail compared with the questions that matter most: the surgeon's specialty training and experience with noses, how honestly the limits are explained, and whether realistic outcomes are described rather than promised. Be cautious of anyone guaranteeing a particular look. A considered answer about the approach and the recovery is reassuring; a quick upsell of a technique is not.
